Job Title: Coordinator/Case Manager

The Coordinator/Case Manager shall be responsible for assisting in all areas necessary for the development, well-being and security of the clients. The Coordinator/Case Manger shall report directly to the Executive Director.
Duties and Responsibilities:
· Carrying a case load of approximately 25 clients (12 Next Step and 12-15 HOPWA)
· Must be familiar with mental health and HIV clientele.
· Preparing well written, legible, and understandable progress notes on an on-going and consistent basis
· Ensure that all proper documents are in client charts, that they are kept current and in an organized manner.
· Attend monthly CoC/CES Meetings
· Make daily visits to supportive housing units to ensure that they are well kept and that clients are maintaining their stability.
· Meet with Executive Director monthly to discuss and review entire case load.
· Make rounds to supportive housing units every other weekend.
· Outreach to the mental health day program where client is receiving treatment and meet with treatment team to discuss client progress.
· Transport clients to and from groups once per week.
· Assist with the moving process for incoming and outgoing clients.
· Ensure that each client is instructed in the life skills needed to maintain stability.
· Advocate on behalf of clients as needed.
· HMIS data entry and annual trainings
· Flexible for unexpected changes, issues and emergencies as needed.
· Reports directly to Executive Director. Ability to work some nights, weekends, and holidays.
· Ability to work with various age groups and clients with various needs.
· Be willing to venture and extend self to assist others.
· Demonstrate sensitivity to the issues and concerns of the populations served by the agency.
· Be flexible for a wide range of tasks from the routine and predictable to the unique and unexpected.
· Regular attendance and punctuality expected.
THESE DUTIES AND RESPONSIBILITIES ARE SUBJECT TO CHANGE AS DEEMED APPROPRIATE AND NECESSARY BY PROGRAM COORDINATOR AND SENIOR STAFF MEMBERS
QUALIFICATIONS
1. Bachelor’s degree in related field (Human Service field preferred.)
2. Social Services experience.
3. Valid New Jersey driver’s license

- Salary: $50,000.00-$55,000.00
