Job Title: Case Manager Family Unit

CASE MANAGER: EMERGENCY SHELTER

The Case Manager of the Emergency Shelter Program shall be responsible for assisting in all areas necessary for the health and security of the clients and securing safe, decent affordable housing. The Case Manager shall report directly to the Program Director.

Duties and Responsibilities:
• Assist the Program Director in the client intake process.
• Enforce agency rules and policies.
• Coordinate referrals.
• Develop individualized service plans with residents, identifying goals and barriers, focusing on housing.
• Provide one-on-one weekly meetings, progress tracking and supportive counseling.
• Provide weekly assessment of client’s progress towards goals.
• Keep daily records of the programs and activities of the clients.
• Help to connect clients to needed resources pertaining to health and income.
• Assist clients in locating and securing permanent housing.
• Maintain clients’ files and keep updated case notes.
• Enter client information into HMIS
• Attend program meetings
• Participate in annual workshop/trainings.
• Execute a therapeutic and empathetic approach with clients.
• Communicate with outside vendors.
• Provide follow-up assistance as needed.
• Ability to work some nights, weekends, and holidays.
• Ability to work with various age groups and clients with various needs.
• Be willing to venture and extend self to assist others.
• Demonstrate sensitivity to the issues and concerns of the population served by the agency.
• Be flexible for a wide range of tasks from the routine and predictable to the unique and unexpected.
• Regular attendance and punctuality expected.

THESE DUTIES AND RESPONSIBILITIES ARE SUBJECT TO CHANGE AS DEEMED APPROPRIATE AND NECESSARY BY PROGRAM DIRECTORS AND SENIOR STAFF MEMBERS

QUALIFICATIONS
1. BA in Social Work, Psychology or Human Service field
2. Social Services experience.
3. Valid New Jersey driver’s license.
4. Computer literate
5. Ability to maintain boundaries and handle confidential, sensitive information.
6. Bilingual a plus.

Job Title: Full Time
Schedule: Monday-Friday 8AM to 4PM
Salary: $48,000.00 – $50,000.00
